Background: Clinical genomic implementation studies pose challenges for informed consent. Consent forms often include complex language and concepts, which can be a barrier to diverse enrollment, and these studies often blur traditional research-clinical boundaries. There is a move toward self-directed, web-based research enrollment, but more evidence is needed about how these enrollment approaches work in practice. In this study, we developed and evaluated a literacy-focused, web-based consent approach to support enrollment of diverse participants in an ongoing clinical genomic implementation study. (...) Methods: As part of the Cancer Health Assessments Reaching Many (CHARM) study, we developed a web-based consent approach that featured plain language, multimedia, and separate descriptions of clinical care and research activities. CHARM offered clinical exome sequencing to individuals at high risk of hereditary cancer. We interviewed CHARM participants about their reactions to the consent approach. We audio recorded, transcribed, and coded interviews using a deductively and inductively derived codebook. We reviewed coded excerpts as a team to identify overarching themes. Results: We conducted 32 interviews, including 12 (38%) in Spanish. Most (69%) enrolled without assistance from study staff, usually on a mobile phone. Those who completed enrollment in one day spent an average of 12 minutes on the consent portion. Interviewees found the information simple to read but comprehensive, were neutral to positive about the multimedia support, and identified increased access to testing in the study as the key difference from clinical care. Conclusions: This study showed that interviewees found our literacy-focused, web-based consent approach acceptable; did not distinguish the consent materials from other online study processes; and valued getting access to testing in the study. Overall, conducting empirical bioethics research in an ongoing clinical trial was useful to demonstrate the acceptability of our novel consent approach but posed practical challenges. (shrink)

The evident multi -interpretability of a text causes it gradually to lose its capacity to function as arbiter in the historical debate. It is necessary to define a new link with the past based on a complete and honest recognition of the position in which we now see ourselves placed as historians. In recent years, many people have observed our changed attitude towards the phenomenon of information. For (...) postmodernism, science and information are independent objects of study which obey their own laws. Language and art are not situated opposite reality but are themselves a pseudo-reality and are therefore situated within reality. Because of the relation between the historiographical view and the language used by the historian to express his view - a relation which nowhere intersects the domain of the past -historiography possesses the same opacity and intensional dimension as art. The essence of postmodernism is precisely that we should avoid pointing out essentialist patterns in the past. There is reason to assume that our relation to the past and our insight into it will in future be of a metaphorical nature rather than a literal one. (shrink)

Ob was Mythos im ursprünglichen oder anfänglichen Sinn heissen soll aus den zahlreichen späteren und sich oft widersprechenden Deutungen überhaupt noch ans Licht zu ziehen ist lässt sich fragen. Desgleichen, ob es das Sein selbst ist, das sich im Mythos (als auslegende Seinserzählung) dem archaischen Menschen kund tut oder ob es vom Menschen als dem „interpretations- und orientierungsbedürftigen Wesen” angesprochen wird. Im ersten Falle lässt Mythos sich am besten als Manifestation, im zweiten als Interpretation auffassen. Aber auch wenn es das (...) Sein selbst sein sollte, das den Menschen im Mythos anspricht so ist es auf ihn als das Wesen, das ausspricht dass es angesprochen wird oder sich angesprochen weiss immer noch angewiesen. Die immer von aktuellen Fragen inspirierte wissenschaftliche und philosophische Forschung wird mit der Schwierigkeit konfrontiert, dass die Suche des Mythos „an sich” immer davon auszugehen hat was der Mythos „für uns” bedeutet und dem unmöglich los wird. Wenn Myhos mit einem gewissen Recht als „vorgängiges Seinsverständnis” qualifiziert wird kann mit gleichem Recht gelten, dass jede theoretische Interpretation des Mythos als Artikulierung von etwas das „vorgängiges Mythosverständnis” genannt werden kann aufzufassen ist. Der Grund, der im Mythos gelegt wird, lässt sich nicht mehr „interpretationsfrei” zurückholen. Zugleich zieht er uns immer wieder zu sich zurück. The idea of absolute goodness and the idea of an absolute requitement tend nowadays to be viewed with suspicion in the world of English-speaking philosophy. The tendency is well rooted and has not just arisen by osmosis from the temper of the times. There are various lines of thought, all of them attractive, by which a recent or contemporary academic practitioner of the subject could have been induced into scepticism about an ethics of absolute conceptions.

In the autumn of 1912, C. G. Jung, then president of the International Psychoanalytic Association, set out his critique and reformulation of the theory of psychoanalysis in a series of lectures in New York, ideas that were to prove unacceptable to Freud, thus creating a schism in the Freudian school. Jung challenged Freud's understandings of sexuality, the origins of neuroses, dream interpretation, and the unconscious, and Jung also became the first to argue that every analyst should themselves be analyzed. Seen (...) in the light of the subsequent reception and development of psychoanalysis, Jung's critiques appear to be strikingly prescient, while also laying the basis for his own school of analytical psychology. THIS PAPER INVESTIGATES THE LAWS OF MOTION in Newton and Descartes, focusing initially on the first laws of each. Newton's first law and Descartes' first law were later conjoined in the minds of philosophic interpreters in what thereafter came to be called the law of inertia. Our analysis of this law will lead to the special significance of Newton's third law, and thus to a consideration of the philosophical implications of Newton's three laws of motion taken as a whole. This (...) paper also involves backward glances at an earlier tradition of physics: How do the laws of motion compare with certain basic elements of the philosophy of nature? Principal results and conclusions are: There is no one law of inertia; rather, Newton's first law is distinct from Descartes' in its implications for our understanding of nature. The concept of force entailed by Newton's three laws of motion is not the same as cause of motion as we ordinarily experience and understand it. Newton's three laws, unlike Descartes', do not rule out internal causes of motion in bodies, and are, therefore, compatible with the traditional definition of nature. The motor causality principle, often stated as "all that is moved is moved by another," is not as such incompatible with Newton's three laws of motion. (shrink)
